How we can help
The purpose of Preparing for Adulthood is to support families with a young person with additional needsor a disability, get ready for adult life, to help them become independent, and know where to get help if they need it. We work with families and young people from age 14 to 25.
Young people are encouraged to share their own thoughts and plans for their future with their family, and other adults supporting them.
Planning early gives the young person time to make good choices
As a PFA Navigator I can provide information and support in the following areas
- Navigating local service provision and activities – Signposting families to other specialists and support avenues.
- Employment, Learning and Higher Education – Signposting families to support services, advising families on education options, supporting families with school meetings and EHCP reviews, supporting school and work placements make reasonable adjustments.
- Good Health – Signposting families to relevant services, discuss strategies and coping mechanisms with families.
- Friendships, Relationships and Community Inclusion – Provision of social groups and activities, workshops and information for family members and the young person to aid understanding of conditions.
- Independent Living/Housing Options – Such as supported living, and general support and signposting for young people with housing difficulties
- Life Skills – Sessions focusing on core life skills such as: cooking, cleaning, and money management.
